Temporary Parking Placards
The Illinois Secretary of State's office provides temporary 90-day parking placards to residents with disabilities. Residents must complete the form provided by the Illinois Secretary of State. The individual with a disability and their physician, who certifies to the disability, must complete the application.
A completed application and all required documentation can be mailed to:

Temporary placards can be obtained immediately from the Finance Department at City Hall with the completion of the application. No fee is required.
